Chirurgeon's Forms
Like any organization, particularly a medieval one, we can do nothing without paperwork. There are certain forms and paperwork required to be completed whenever we act as Chirurgeons. The requirements for this paperwork are derived from section IV of the 2004 Chirurgeon's Handbook (with 2007 revisions) the requirements from the Chirurgeon's Handbook are in turn derived from the copora, by-laws, and corporate policies of the Society for Creative Anachronism, Inc.
The following table is a listing of the forms used by Chirurgeons in the Kingdom of An Tir. Each of the forms can be downloaded by clicking on the appropriate document type image in the right hand side of the table.
